Print and complete all of the forms inside, and send the completed forms to:

Before sending in your Broker Kit, please be sure these steps have been completed:

1. Sign both of the Agent Agreements.
2. Enclose a copy of your Agent (and/or) Agency Licenses for Life and Health.
3. Submit the appropriate Appointment Fee (and form) for each state where you wish to sell coverage, other than your resident state. Make check payable to: United States Life Insurance Co.
4. Complete the Assignment of Commission sheet if you wish that your commissions be paid to someone other than yourself.
5. Complete the W-9 form, using the Instructions.

IMPORTANT: Please remember, if the forms are missing any information, or your Appointment Fee and a copy of your License are not attached, we will return the original forms for completion. At that time, we will ask you to return the completed forms within two weeks. This process will ensure we are properly servicing you, our new agent.
